Please complete the form in its entirety. Filling out this form is not mandatory but it will make your visit much shorter as we will be able to enter the information required by the FDLE and FBI prior to your arrival. Please try to submit the form 24 hours or more in advance of the scheduled visit.

All information is essential and required in order to complete your background check or fill out your card (fingerprint cards used to send out of State for employment and other reasons). This information is strictly confidential. Once we do the fingerprinting, the information is sent directly by us to the FDLE.

We do not get the results and neither do you. Typically, it takes 48 to 72 hours for the State to process but it can take longer if something is found and requires further investigation (non-card live scans only). Our obligation is to fill out the information, do the fingerprint scan and submit it as soon as possible. We do get confirmation that they received it (usually within minutes) and also when they have completed the search on their end.

The line containing a request for the "ORI" number is an essential tool for the State to direct the results to the proper authority, i.e., Real Estate Board, Liquor License Board, Health Care licensing administration, Contractor licensing and Education, Volunteers, Child Care (DCF) and many other organizations tied to the reason for your background check.This is the line where you would enter a VECHS number if you have that instead of an ORI number. If you do not have an ORI number it is possible (with a discussion) we can determine what that number should be. However, if we assist you in providing that number (based on information provided by you) and for some reason it is incorrect, it is possible that you will be responsible for a re-submission and there could be a second charge in that instance. Newly licensed CNA's and HHA's may inquire about 30 day financing of their associated scan fee. Please call in advance.
